Plantar Fasciitis, Plantar Fasciopathy or Plantar Heel Pain: What's the Difference?

Pain under the heel, particularly during the first few steps in the morning or after a period of rest, is one of the most common foot complaints. If you've experienced these symptoms, you've likely heard the term plantar fasciitis.
However, depending on who you ask, you may also hear plantar fasciopathy or simply plantar heel pain. While these terms are often used interchangeably, they each reflect a slightly different understanding of the condition.
So, what's the difference, and does it actually matter?
Plantar fasciitis vs plantar fasciopathy
The plantar fascia is a thick band of connective tissue that runs from the heel bone to the toes, helping to support the arch of the foot and absorb forces during walking and running.
Traditionally, pain in this area was labelled plantar fasciitis. The suffix -itis suggests inflammation, and for many years inflammation was thought to be the primary cause of symptoms.
As research has evolved, we've learned that persistent plantar heel pain often involves changes in the structure and function of the tissue that aren't explained by inflammation alone. Instead, there appears to be a combination of altered tissue quality, reduced capacity to tolerate load, and changes in how the body responds to stress. For this reason, many clinicians now prefer the term plantar fasciopathy for longer-standing cases.
That doesn't necessarily mean inflammation plays no role—particularly in the early stages—but it is unlikely to explain the whole picture.
Why "plantar heel pain" is becoming more common
Many healthcare professionals now use the broader term plantar heel pain.
This recognises that pain beneath the heel isn't always caused solely by the plantar fascia. Other structures, such as the heel fat pad, surrounding nerves, or nearby joints, can also contribute to symptoms. In many cases, several factors may be involved at the same time.
From a treatment perspective, understanding what is contributing to an individual's symptoms is often more useful than focusing solely on the label.
What do we know about plantar heel pain?
Although there is still much to learn, research has identified several factors that may contribute to plantar heel pain.
These can include:
A sudden increase in walking, running or standing.
Reduced strength or endurance of the calf and foot muscles.
Changes in ankle mobility.
Footwear changes.
Higher body weight.
Age-related changes in tissue capacity.
Hormonal changes during perimenopause and after menopause.
Individual biomechanics and activity demands.
Not everyone develops plantar heel pain for the same reasons, and it's often a combination of factors rather than a single cause.
So, what treatment works best?
This is where the evidence becomes particularly interesting.
A wide range of treatments—including strengthening exercises, stretching, taping, orthotics, shockwave therapy and education—have all shown benefits in research. However, no single treatment has consistently been shown to work best for everyone.
This is likely because plantar heel pain is not the same condition in every person. The factors contributing to symptoms can differ considerably from one individual to another.
For that reason, current best practice is less about finding one "magic" treatment and more about identifying the factors contributing to each person's symptoms. An assessment can help identify areas such as strength, mobility, loading patterns, footwear, activity levels or other contributing factors that may be influencing recovery.
Treatment can then be directed towards addressing those specific deficits, rather than relying on a one-size-fits-all approach.

Whether it's called plantar fasciitis, plantar fasciopathy, or plantar heel pain, the terminology reflects our evolving understanding of this common condition.
Perhaps the most important message is that heel pain is often multifactorial, and there is rarely a single explanation—or a single solution. While many treatments can be effective, the best approach is usually one that considers the individual factors contributing to a person's symptoms and uses these to guide management.
